Job Description

CMA Role Purpose:In an effort to become a national leader in quality and to ensure our patients are engaged in their care, the centralized Primary Care Support (PCS) team was established to assist the clinic providers and staff with reminding patients of needed care for specific chronic diseases and preventative screenings. Through proactive management of these patients, we will be reducing the likelihood of them developing long term complications. The CMA/RMA Primary Care Support Assistant is responsible for performing quality improvement tasks, including but not limited to, patient chart review, contacting patient to remind them of needed services, placing orders so patient can complete the needed test/screening, documentation in the electronic medical record, and scheduling of appointments.LPN Role Purpose:In an effort to become a national leader in quality and to ensure our patients are engaged in their care, the centralized Primary Care Support (PCS) team was established to assist the clinic providers and staff with reminding patients of needed care for specific chronic diseases and preventative screenings. The LPN- Primary Care Support Assistant is responsible for performing quality improvement tasks, including but not limited to, patient chart review, contacting patient to remind them of needed services, provide protocol based assessment for symptoms, placing orders so patient can complete the needed test/screening, documentation in the electronic medical record, and scheduling of appointments.Dean Clinic offers a one time sign on bonus of $2500.00 to part time and full time Medical Assistant, LPN and Paramedic new hires.CMA Responsibilities:1.Contact patients who are due for needed services.2.Engage patients to be proactive in their care.3.Educate patients on the importance of these tests/screens.4.Appropriately document completed work per department workflow. 5.Learn and follow documented workflows.6.Maintain patient contact tracking documents. 7.Place orders in Epic per protocols.8.Accurately schedule and/or cancel patient appointments as indicated per protocols.9.Update patient electronic medical record as appropriate.10.Work as an extension of clinic practices to help provide patient health reminders.11.Offer feedback to help improve processes to increase patient satisfaction and staff efficiency.12.Update patient demographic information within Epic system as indicated per established protocols.13.Attend meetings and in-services as required.14.Comply with Dean policies regarding HIPAA.15.Keep records appropriate to area and perform other clerical functions as needed.16.Be alert and prepared to deal with circumstances that require service recovery.17.Act as a resource for patients and/or internal customers.18.Perform delegated nursing or medical acts beyond basic nursing care under the direct supervision of the RN/Medical provider.19.Perform other duties as required.LPN Responsibilities:1.Contact patients who are due for needed services.2.Engage patients to be proactive in their care.3.Assess patients as needed per protocol workflow under the direct supervision of the RN.4.Connect symptomatic patients to RN for further assessment.5.Educate patients on the importance of these tests/screens.6.Appropriately document completed work per department workflow7.Learn and follow documented workflows.8.Maintain patient contact tracking documents. 9.Place orders in Epic per protocol.10.Accurately schedule and/or cancel patient appointments as indicated per protocols.11.Update patient electronic medical record as appropriate.12.Work as an extension of clinic practices to help provide patient health reminders.13.Offer feedback to help improve processes to increase patient satisfaction and staff efficiency.14.Update patient demographic information within Epic system as indicated per established protocols.15.Attend meetings and in-services as required.16.Comply with DMG policies regarding HIPAA.17.Keep records appropriate to area and perform other clerical functions as needed.18.Be alert and prepared to deal with circumstances that require service recovery.19.Act as a resource for patients and/or internal customers.20.Performs delegated nursing or medical acts beyond basic nursing care under the direct supervision of the RN/medical provider.21.Performs other duties as required.
LPN Required: 1.High school diploma or equivalent and graduate of an approved Licensed Practical Nurse program.2.Current State Licensure as a Licensed Practical Nurse (LPN)3.Previous healthcare experience.4.Excellent critical thinking skills.5.Be available to work afternoon and evening hours. 6.Pleasant, outgoing personality.7.Excellent organizational, written and verbal communication skills.8.Ability to perform multiple tasks in a busy, ever-changing environment.9.Ability to shift attention easily and quickly to meet patient needs.10.Previous personal computer training/experience.11.Excellent telephone skills/etiquette.12.Fundamental desire and ability to provide exceptional customer service.Preferred:1.Experience in a primary care setting.2.Experience with Epic.3.Previous experience with Microsoft Access and Microsoft Excel.4.Experience in quality improvement or process improvement.Essential Physical Functions:The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. Acceptable credential includes Registered Medical Assistant (RMA through the AMT Organization only) and Certified Medical Assistant (CMA).3.Previous healthcare experience.4.Be available to work afternoon and evening hours.5.Pleasant, outgoing personality.6.Excellent organization, written and verbal communication skills.7.Ability to perform multiple tasks in a busy, ever-changing environment.8.Ability to shift attention easily and quickly
